If you wish to refer a patient to us please download and complete the Veterinary Consent form and return it back to us, or alternatively email info@cchydrotherapy.co.uk and give us authorisation to treat your patient, please ensure to include the clinical history and any other relevant details.

​All Therapists at Country Canines Hydrotherapy are well qualified and experienced which enables you to have confidence in us when referring your patients for hydrotherapy rehabilitation treatment. If you are unsure about any of our treatments or the suitability of treatments for your patient(s) please do not hesitate to contact us directly and we will be happy to discuss any cases with you.
